Changes to Personal Details

Please help us to keep your personal information up to date by advising the Scheme administrators (Broadstone Consultants & Actuaries Ltd) of:

 Address change

 Changing church (if you are a minister) - please complete the below Change of Pastorate form where required

Change of Pastorate Application Form (October 2018) 

 Leaving employment (if you are a member of the Staff Section or the Basic Section)

 Up to date Expression of Wish Form

 If you get married/divorced, particularly if you change your name

 If you want to be contacted by email, whenever your email address changes

For name or address changes, you will also need to tell Legal & General. Their contact details will be on the letter they sent you after setting up your pension account, or you can go through their website, or telephone 0345 070 8686 and enter pin number 97 when prompted by the recorded message.  The person who answers your call will then know that you are ringing about the Baptist Pension Scheme.
